REGISTRATION FORM, CONTRACT, WAIVER FOR COUNTRY HOME PET RETREAT, LLC:

THE CLIENT AGREEMENT IS DUE BY THE START OF SERVICE & SHOULD BE SUBMITTED PRIOR TO DROP OFF - A MANDATORY REQUIREMENT & NO EXCEPTIONS

Please complete this agreement to the best of your knowledge, email proof of vaccination records (mandatory requirement and must be in the form of an official veterinary record), and execute with your electronic signature.

Complete the agreement once and Country Home Pet Retreat, LLC will keep your client agreement on file for all future service. If your client agreement requires updating due to a new pet, change in pet and/or change in pertinent information, etc. You can update your client agreement information by submitting a new updated version of the client agreement at any time.

*Please also fill out a separate pet info form for each of your pets.

THIS AGREEMENT INCLUDES A RELEASE OF LIABILITY AND WAIVER OF LEGAL RIGHTS AND DEPRIVES YOU OF THE RIGHT TO SUE COUNTRY HOME PET RETREAT, LLC, THE OWNER, OR RELATED PARTIES. DO NOT SIGN THIS AGREEMENT UNLESS YOU HAVE READ IT IN ITS ENTIRETY.

THIS AGREEMENT is entered into by and between Country Home Pet Retreat, LLC and the undersigned hereinafter referred to as "owner".

1) I (Owner) represent that I am the legal owner of the above named dog(s) and I assume all risks, dangers, and responsibility for injuries to the named dog(s). Owner understands and agrees that Owner is solely responsible for any harm while Owner’s dog(s) is/are attending Daycare / Boarding / Training.

2) PHOTO AND VIDEO RELEASE: We love to post pictures and videos on social media, our website, and/or in printed ads. Owner agrees to allow Country Home Pet Retreat, LLC to use Owner’s pet’s name and any images or likeness of Owner’s pet taken while he/she is at Country Home Pet Retreat, LLC, in any form, for use at any time, in any media, marketing, advertising, illustration, trade or promotional materials without compensation, and Owner releases to Country Home Pet Retreat, LLC all rights that owner may possess or claim to such image, likeness, recording, etc.

3) DOG’S HEALTH: Owner further understands and agrees that Owner’s dog(s) are healthy and will at all times while attending Country Home Pet Retreat, LLC have a current vaccination for rabies. Owner is not enrolling any dog that has any condition that could potentially jeopardize the health of other dogs or people and has not had any potentially communicable condition within 30 days prior to enrollment. Owner further understands that even if the owner’s dog(s) is vaccinated for Bordetella (Kennel Cough) there is a chance that the Owner’s dog can still contract Kennel Cough. Owner agrees that owner will not hold Country Home Pet Retreat, LLC responsible if the owner’s dog(s) contracts Kennel Cough or other dog-dog transmitted ailments.

4) VACCINATIONS: ALL VACCINES HAVE A MANDATORY 14 DAY WAITING PERIOD AFTER ADMINISTRATION IN ORDER TO ATTEND &/OR START SERVICE. The 14 day waiting period is a general standard for the industry. This is due to potential virus shedding, and also to ensure the vaccination has had the time to take full effect prior to the start of any service.

5) FLEA FREE: All dogs must be flea free. Should a dog be observed to have fleas, a flea bath treatment will be applied and a bathing and cleaning fee will be charged to the owner at a rate of up to $80.00 depending on severity and size of dog.

6) GROUP PLAY: All dogs must pass a general behavior assessment in order to attend dog group play. No dog may be admitted to group play who shows any signs of aggression. Owner further understands and agrees that in admitting owner’s dog(s) to group play, Owner is representing to Country Home Pet Retreat, LLC that Owner’s dog(s) is/are in good health and have not harmed, shown aggression, or exhibited any threatening behavior towards any person or other animals. Any dog demonstrating aggression or other behavior deemed unacceptable by Country Home Pet Retreat, LLC staff may be required to leave daycare/boarding.

7) RELEASE OF LIABILITY: Owner understands and agrees that during normal dog play, Owner’s dog may sustain injuries. Dog play is monitored by Country Home Pet Retreat, LLC staff to best avoid injury, but scratches, punctures, torn ligaments, and other injuries may occur despite the best supervision. Owner further understands and agrees that neither daycare/boarding nor any of its employees, staff, or volunteers will be liable for any illness, injury, death, and/or escape of Owner’s dog(s) provided that reasonable care and precautions are followed, and Owner hereby releases Country Home Pet Retreat, LLC staff or volunteers of any liability of any kind whatsoever arising from or as a result of Owner’s dog(s) attending Country Home Pet Retreat, LLC.

8) PERSONAL PROPERTY: Owner understands that Owner is solely responsible for any harm, including to any other pet(s), to the employees or invitees of Country Home Pet Retreat, LLC, or to the equipment, facilities, or other property of Country Home Pet Retreat, LLC, caused by Owner’s dog(s). Owner also agrees that Country Home Pet Retreat, LLC shall not be responsible or liable for any lost or damaged personal property belonging either to Owner or Owner’s pet.

9) FOOD when boarding overnight, Owner agrees that it is the Owner’s responsibility to leave an adequate supply of food and medications for Owner’s dog(s) during the entire time Owner’s dog is cared for by Country Home Pet Retreat, LLC. Should the food/medication supply need replacement, Owner authorizes Country Home Pet Retreat, LLC to purchase replacement and Owner will reimburse Country Home Pet Retreat, LLC for the actual food and medication costs plus up to $50.00 time/distance trip fee (per occurrence).

10) MEDS: All medications to be administered must come in their original RX packaging with the correct prescribed dosing. Owner agrees to assume all risk associated with the administration of medication by Country Home Pet Retreat, LLC.

11) VETERINARIAN LIABILITY AND CARE: Owner agrees to allow Country Home Pet Retreat, LLC to obtain medical treatment for Owner’s dog(s) if he/she appears ill, injured, or exhibits any other behavior that would reasonably suggest that dog(s) may need medical treatment including anesthesia. Owner agrees to be fully responsible for the cost of any such medical treatment and for the cost of any transportation for the purposes of such treatment. Owner gives permission to Country Home Pet Retreat, LLC to use Owner’s vet or nearest vet hospital if unavailable for required treatment.

12) WEAKENED IMMUNE SYSTEM: Owner understands special-needs dogs, young puppies, and senior dogs naturally have a higher risk of injury, stress-related illnesses, weakened immune system, or exacerbation of any pre-existing condition. As such, by using our facility for daycare or boarding the Owner is waiving any claim for injury or illness experienced by the Owner's dog while in our care.

13) ELDERLY DOGS: Owners of elderly dogs (approaching the end of life) need to know if in the event Owner’s dog passes on while in our care, our policy is to transport Owner’s dog to Owner’s vet (within 30 miles) where he/she will be held until Owner can be reached by the Vet for further instructions. If Owner’s vet is closed or outside of our transport distance, then we will transport to the nearest veterinary facility. Trip charge will apply based on time/distance.

14) GROOMING: expense to be paid if Owner’s dog(s) nails are too long and cause breaking of skin or deep scratches to staff, volunteers, and/or other dogs. Nail trim cost up to $30.00. In addition if Owner’s dog(s) happens to soil their fur we will bathe them at Owner’s expense.

15) PAYMENTS/RETAINERS/REFUNDS: A 50% retainer is due at time of booking. Remainder of total is due one week prior to check in for that booking. If booking is within one week prior to check in, full payment will be due at time of booking. An invoice will be sent to you.

16) CANCELLATION / NO-SHOW POLICY: If a stay is canceled, it is up to Country Home Pet Retreat, LLC discretion if an up to 50% credit of all fees paid towards booked stay can be applied to future stays. No refunds will be granted. No call/no show’s will not be eligible to be credited any amount and will be charged full payment at any future bookings that will be entirely non-transferable and non-refundable.

17) CLOSING TIME/PICK UP: Owner agrees that if Owner’s dog(s) is/are not picked up by the agreed upon date and time, then Owner hereby authorizes additional boarding for dog(s) and to pay Country Home Pet Retreat, LLC for applicable boarding charges at (1.5x times original rate) plus any charge for required food.

18) RIGHT OF REFUSAL: Country Home Pet Retreat, LLC reserves the right to refuse admittance to any dog or dismiss any dog that does not meet or maintain the health, temperament or other daycare/boarding standards. The determination shall be made at the sole discretion of Country Home Pet Retreat, LLC. If for any of these reasons a dog is required to be dismissed during an active stay, Country Home Pet Retreat, LLC will contact owner first, emergency contact second, to schedule someone to come and pick up the dog(s). If no one is able to be reached the dog will be transported to the nearest veterinary facility and boarded there until owner can be reached. Owner agrees to be fully responsible for the cost of any such requirements.

19) ABANDONMENT OF ANIMALS: Owner understands if they do not pick up their dog(s) within 5 days after the agreed upon pick up date, a certified letter will be sent to the owner’s address as described in the registration form regarding the animal’s abandonment. Should the animal not be removed within the specified time as described in the certified letter, I, the owner described in the registration form submitted to the facility, hereby relinquish all claims to the animal, but the owner understands that he/she is NOT RELIEVED OF THE CONTRACTUAL OBLIGATION TO PAY FOR ANY NECESSARY TREATMENT, BOARDING OR CARE FURNISHED BY THE COUNTRY HOME PET RETREAT, LLC.

20) Drop-Off/Pick-Up Appointments: We run by appointments only. All drop-off and pick-up times must be scheduled in advance. No drop-ins are allowed. Same day appointments may be available, but still need to be scheduled. Please be prompt and on time. If you are running more than 10 minutes early or late, please text or call to let us know. If you are more than 20 minutes late to an appointment, it may be rescheduled or canceled. We strive to be flexible and available when you need us. So if anything changes, please just notify us as soon as possible and we will try to get you rescheduled as soon as possible.

21) FULL FORCE AND EFFECT: Owner further understands and expressly agrees that each and every of the foregoing provisions contained in paragraphs 1-21 shall be in force and effect and shall apply to each and every occasion on which Owner’s dog(s) stays with Country Home Pet Retreat, LLC for daycare, boarding, or other services, as the case may be. This agreement shall remain in full force and effect as between the parties until and unless otherwise canceled or superseded by a writing signed by the parties. Owner hereby certifies that Owner has read and understands this Waiver and Release of Liability and the regulations set forth above. By signing this agreement, Owner agrees to be bound by its terms and conditions.

Veterinary Instructions and Release Form:

This form will be retained on file and will be used to authorize veterinary treatment in the event that your pet(s) require treatment during your absence, while in our care, and we are unable to contact you at the time. Should you change veterinarians please notify Country Home Pet Retreat, LLC.

Country Home Pet Retreat, LLC will make every effort to use your primary veterinarian or veterinary clinic. However, if it is not practical to do so, Country Home Pet Retreat, LLC reserves the right to utilize the services of any available emergency veterinary clinic. The following information will be helpful if the clinic we utilize requires documentation from your primary clinic.
